Forgot Your Password?
Hoag Medical Group (HMG) is using MyChart to help you access, manage, and protect your online health information using a single email and password. You can use this email and password anywhere you see the “Log in to Hoag Connect MyChart” button, including patient portals and other apps you use to manage your health records from various healthcare providers.
First Time Logging In?
If this is your first login because you don’t yet have a Hoag Connect MyChart account, please refer to your healthcare provider’s instructions for registering for their patient portal and you will be walked through creating MyChart account as part of that process.
If you share information from the Hoag Connect MyChart patient portal with another individual, you acknowledge and accept responsibility for your decision to provide them access to potentially sensitive information.
However, If you learn or suspect that your username or password has been wrongfully used or disclosed, you should promptly notify us and immediately reset your password. To help ensure the security of your password or account, please sign out of your account at the end of each session
How to Reset Your Hoag MyChart Password
- Go to the Hoag MyChart login portal at https://www.hoagconnect.org/MyChart/Authentication/Login?
- Click “Forgot Password?”
- Verify your personal information
- Enter your Username
- Enter your Date of Birth
- Enter the E-mail Address that you used
when creating your MyChart account,
click “Next”
- Answer your Security Question, click “Next”
- Enter your new password twice, click “Next”
- Enter your MyChart Username
- Enter your Birthdate (mm dd yyyy)
- Enter the E-mail Address you used when creating the account (abc@xyz.com)
- Click the blue NEXT button.
- Insert your security answer. (It is not case-sensitive.)
- Click the blue NEXT button
- Enter your new password twice.
- Click the blue NEXT button.
Hoag MyChart Frequently Asked Questions
I forgot my username or password. What should I do?
If you’re having trouble logging in, click the “Forgot Username?” or “Forgot Password?” link below the log in fields for assistance. You will go through two-step verification to verify your identity so you can recover your username or password.
I didn’t receive my two-step verification code. What should I do?
Try checking the spam or junk folders in your email. If the email with your code is not there, try clicking Resend Code. If you still didn’t receive the email, it could be that your account has a different email address on file.
If you have multiple email accounts, check one of the others to see if the verification code was sent there instead. If none of your accounts received the email, it could be that we don’t have an email address on file. If this is the case, call our MyChart Patient Support Line at 877-464-2778 for quick support.
I was logged out of MyChart. What happened?
We aim to protect your privacy and information. If you remain idle for 10 minutes or more after you log in to MyChart, you will be automatically logged out. We recommend that you log out of MyChart if you need to leave your computer for even a short period.
What do I do if I get locked out of my account?
To have your account re-activated, send an email request to Member.Services@hoag.org or call our MyChart Patient Support Line at 877-464-2778.
I have multiple MyChart accounts. How do I link them together?
If you’ve been seen at another healthcare organization, you might be able to view information from that medical record right in MyChart. You might have heard of this feature referred to as Happy Together. This includes information from other organizations, such as:
- Allergies
- Care team
- Health issues
- Medications
- Messages
- Test results
- Visits
How do I delete my account?
You can request that we deactivate your account by contacting the MyChart Patient Support Line at 877-464-2778.
